SwanCare Group is committed to the protection
of personal and sensitive information that is provided to
us from any source. SwanCare Group is governed by the Aged
Care Act 1997, its own policies and procedures and the Commonwealth
National Privacy Principles. This includes SwanCare Group's
Privacy Protection Principles which have been designed to
ensure that personal information is protected.

SwanCare Group stores personal information
that has been provided by residents, residents' families,
job applicants, banks, Government departments, treating health
practitioners and care staff. The primary purpose for collecting
and storing this information is to process applications for
residency and in the on-going provision of health and accommodation
services while in our care.

We use this information to assist us in
providing health and accommodation services to you as a resident.
From time to time it may be necessary to disclose and share
your personal and sensitive information with others, such
as treating doctors and allied health professionals. This
is done strictly within the privacy guidelines.

In accordance with the current guidelines,
your consent will not be sought when using your personal and
sensitive information for the provision of health care services
but when information is sought that is not health related,
we will specifically ask for your consent. In these cases,
you will be asked to sign a Consent Form and will be provided
with a privacy information sheet.

SwanCare Group will not use any resident
information for any third party commercial marketing purpose.

As part of our policy, we wish to ensure
that the information held by us remains accurate, complete
and up-to-date. We ask that residents provide us with changes
to your circumstances and request that changes be advised
either to your Care Centre Manager or in the case of Bentley
Park residents, to Administration. We have confidentiality
rules for all of our staff, specific contractors and third
party organisations. Where we no longer have a need for personal
or sensitive information that is held on file, it will be
securely stored for gazetted periods or destroyed.

You may have access to your personal information
upon request to your Care Centre Manager or Administration,
where it will be provided to you as soon as practicable. A
small administrative fee may be charged. In some cases, access
to your information may be restricted or denied under the
current legislation.

Commonwealth identifiers such as tax file
numbers, Medicare numbers and Veteran Affairs information
may be used when it is necessary to provide health services
to you. They will be used for no other purpose.

If you believe that SwanCare Group has
not acted in accordance with this Privacy Policy or the consent
you have given us, you may lodge a complaint to the Chief
Executive Officer in person, by phone, fax, email or in writing.
Your complaint will be treated confidentially and acted on
quickly. Under normal circumstances, a response will be made
to you within 24 hours. If you are not satisfied with the
resolution, you may refer your complaint to the Federal Privacy
Commissioner by telephone to:
